This measure shows the number of clients served, as well as the percent of clients served by race through a series of programs administered by the Cuyahoga County Division of Senior and Adult Services (DSAS) that allow seniors and adults with disabilitiesto remain independent and living in their own homes, as well as other core programs. These programs include:

• Adult Protective Services, a state-mandated program to protect and assist adults 60 and older who may be victims of abuse, neglect, self-neglect, and/or financial exploitation. Allegations of abuse of adults with disabilities ages 18 and over are investigated on a voluntary basis,
• The Centralized Intake Unit provides seamless intake services through the phone number (216) 420-6700 “One Call Does It All.” A web portal is also available to make referrals for Adult Protective Services,
• Community Social Services Program, which provides services through community-based contracts with local municipalities and non-profit organizations. Services include adult day services, adult development, congregate meals, and transportation designated to reduce social isolation and loneliness,
• Home Support, which provides person-centered home care (personal care and homemaking) that helps clients maintain a safe, wholesome environment in their own home at an affordable price,
• Information Services through the Aging and Disability Resource Center (a partnership with the Western Reserve Area Agency on Aging), which provides an array of public benefits to seniors, caregivers, and persons with disabilities including information assistance, benefits assistance and MIPPA (Medicare Improvement for Patients and Providers Act) assistance. Person-centered case management assistance to address complex needs and navigate available resources is also provided,
• Options for Independent Living, which serves older adults and adults with disabilities ages 18 and older who are low income and not yet eligible for any Medicaid Waiver programs. Person-centered services include home-delivered meals, personal care, emergency response systems, homemaker services, chore services, and medical transportation. Minor bathroom modifications are also available. DSAS is Medicare/Medicaid certified, and
• A number of services intended to facilitate a Reduction in Food Insecurity among seniors. Food insecurity is measured by the  number of home-delivered meals, congregate meals, clients who receive SNAP assistance through the Information Services unit, and the DSAS food pantry. According to Feeding America, food insecure seniors are more likely to experience chronic health conditions depression, asthma, chest pains, limitations in activity, and high blood pressure.

Quarterly performance data for each program represents the number of unique clients served during the three-month period. Clients may be counted in multiple quarters depending on the services received. 

Annual performance data for each program represents an unduplicated client count, meaning each client is counted only once regardless of whether they received services in more than one quarter during the year.

Data are reported by the Cuyahoga County Division of Senior and Adult Services on a quarterly basis.

The Division of Senior and Adult Services empowers seniors and adults with disabilities to age successfully by providing resources and support that preserve their independence. These services help seniors and adults with disabilities remain safe and healthy in their own homes, preventing early nursing homes or assisted living placement.

In Q4-20, Cuyahoga County began reporting Cuyahoga Performance data by race and ethnicity when available.  The Older Persons Remaining Independent through Cuyahoga County Programs page has been update to report the percent of clients served through a series of programs administered by the Cuyahoga County Division of Senior and Adult Services by race and ethnicity.  Race and ethnicity data reported on this page by the Cuyahoga County Division of Senior and Adult Services are collected from two case management systems, both of which use national standards for reporting race and ethnicity data.
