To provide a well coordinated system of care for individuals and families experiencing homelessness that leads to better outcomes. A well coordinated system of care ensures uniform assessment and intake, matching client needs with services, and collaboration on local, State, and Federal grants, to achieve the community plan objectives.

When homelessness becomes more rare, brief and non-recurring we will have succeeded. The point in time count results will be released in March 2018 and inform our targets for measurable goals. In addition, TJACH will generate performance data for the local system of care by the end of 2017, which will provide data in the remaining categories.
