The Data source resides in Sweeps. In Sweeps, click on the <DAR> tab, then click on <DAR Service Summary Report>. Fill in the Program selection by clicking on the gray box to the right of <Program> and enter the following codes in the boxes; 11C, 11T, 16F, 16T, 16V, 24H, 36P, 80G and click <close>. Next click on the gray box to the right of <Service Code> and enter the following codes; 1 ,2, 3, 4, 8, 20, 21, 24, 25, 89, 121, 309 and click <close>. In the <starting Date 1> box enter the beginning date of the quarter. In the <Ending Date 1> box enter the last day of the quarter. Ensure <Print Services> is selected. Click OK. Next in Sweeps, click on the <DAR> tab, then click on <DAR Service Summary Report>. Fill in the Program selection with 90T. Then click on the gray box to the right of <Service Code> and enter the following codes; 90, 91, 92, 93 and click <close>. In the <starting Date 1> box enter the beginning date of the quarter. In the <Ending Date 1> box enter the last day of the quarter. Ensure <Print Services> is selected. Click OK. These reports are run quarterly by the CH Superintendent. Additionally, use a daily calendar is used to count the work days available in the quarter that is being calculated, do not include scheduled city approved holidays, Saturdays or Sundays in the total. Finally, count the number of Consumer Health Specialists working during this quarter.
NOTE: As the calculation for this data is complicated and completed within the system, only the average number will be reported.
