Data Source resides in the Consumer Health Division SWEEPS software. Greater than 30 demerits for food establishments (16F) reports can be extracted from the SWEEPS database by selecting: MFR tab and following the tree to Reports > Click on Master File Listing > DAR tab in sequence. Then enter dates in desired date range boxes. After that, click on the gray square to the right of the "Service Code" and enter Service codes 1-routine inspection and 2 - follow up inspection. Then enter Starting score 30 demerits and Ending score 100 demerits in their respective boxes. Click OK to run report.16f After obtaining the # of facilities with failing scores the person needs to pull another report which will give him or her the number of food facilities(16F) inspected (routine and follow up inspections) within the quarter. That can be extracted in SWEEPS by selecting the DAR tab and following the tree to Reports, click on DAR Service Summary. Then, the person extracting the report needs to enter 16F in the Program box and enter the appropriate a data range in the date range boxes. After that, click on the gray square next to "Service Code" and enter the service codes 1 - routine inspection and 2 - follow up inspection in the Service codes boxes then click OK to run the report. The CH Manager or a CH supervisor can run this report. The CH Manager maintains this report.
